|
Under general supervision, following established procedures, provides improvement efforts in the health status of the community through assessment of needs, program planning, provisions of services and evaluation. The emphasis is on health promotion and disease prevention activities. |

1. |
Identify health status of individuals, families and community noting physical, mental, social and/or environmental deviation from normal using nursing skills of interviewing, assessing, nursing diagnosis and intervention. |
|

Coordinates, educates and conducts comprehensive diabetic self-management care one on one in the home and clinic setting. |
|
|
|
|
|
3. |
Provides family planning and promotes general women’s health care. |
|
|
|
|
|
4. |
Conducts monthly and quarterly reports on STDs, as well as monitors and orders all appropriate medication for STDs. |
|

Work closely with Albuquerque STD Director. |

|

6. |
Counsels, teaches, and demonstrates to patient(s) and/or families, either in the home, health center, or community center, in acute or chronic conditions the application of preventative health practices; nursing procedures in care, as well as rehabilitative measures. |

Assesses patients ability to carry out health care and rehabilitative measures. |
|
|
|

|
8. |
Promotes health and illness prevention by teaching individuals or groups in the Health Center, or community. |

9. |
Maintain a working relationship with IHS facilities to provide continuity of care using the referral process. |
|
|
|

|
10. |
Control communicable disease by case identification, follow up and prevention through evaluation of immunization level and referral as indicated. |
|

11. |
Make home visits according to established priorities. |
|
|
|
|
|
12. |
Demonstrates ability to determine health status of individuals, families and to initiate action to meet identified needs. |

Prepares all required IHCRC monthly reports. |

Maintains staff immunizations in a timely manner. Collaborates with department managers to ensure timely compliance. |

Complies with all AAAHC standards and requirements such as annual TB test, CPR and CEU if applicable. |

Knowledge, Skills, Abilities |
|
Graduate of an accredited school of Nursing. RN required. BSN or MSN preferred. |
|
Possess a current and valid Oklahoma nursing license. |
|
CPR certification. |
|
Familiarity with MS Office and/or other relevant computer software. Intermediate computer skills. |
|
Must have a valid Oklahoma driver’s license, good driving record and proof of valid liability insurance. |
|
Broad knowledge of community health nursing principles, practices and procedures. |
|
Knowledge of available resources for coordination of care between hospital, health center, school and community. |
|
Ability to demonstrate a special understanding of Indian culture. Familiarity with Native American health care trends. |
|
Strong written and oral communication skills. Excellent interpersonal skills. |
|
Ability to multi-task and work independently, demonstrating initiative and accuracy in work results. |
Working Conditions:
|
The employee is on feet most of day. Intermittent physical activity including walking, standing, sitting, lifting of light items (up to 10 pounds) such as files, book and instruments, lifting of small children (up to 50 pounds),and supporting patients is essential. This position performs tasks that involve exposure to blood, body fluids, or tissues. Incumbent will be exposed to virus, disease and infection from patients and specimens in working environment. Personal protective equipment use required. Travel outside the clinic may be required. |
